DnD Creature Feature: The Third Eye

This Week’s monster will eerily follow your players, show up at the worst time, like while taking a long rest or after beating a tough boss. It will be visible to them and only them, while everything else goes to hell, but people blame it on other circumstances. The “Third Eye” is hellbent on killing the players for disturbing it, while simultaneosly wanting to be disturbed…

I used this monster as a “planned random encounter” for my players. And that’s basically what this grotesque abberation is: Random, unpredictable and devious.

Feel free to use this encounter setup or create your own:
“As you exit the forest, you come across a small wooden cabin with two key features: It lacks a door and only has one medium window.”

If the players open the window, which is unlocked, or enter the cabin in another way, describe the interior like this:

“The inside of the cabin is dark and dusty, the only piece of furniture is a mirror in one corner, which reflects towards the window. As you look around you notice that the cabin is filled with fancy furniture and heaps of gems and art objects in the reflection of the mirror. And behind your reflection, standing outside of the cabin, staring at you, is a horrific humanoid creature boasting three eyes and a maw that forms teeth out of claylike skin.”

The creature disappears, but will appear to your players whenever they are weak, or in a place like a tavern where they will be seen as akward or dangerous, since the Third Eye is only visible to the characters. From there, you may do whatever you please to haunt your players, and make the Third Eye always seem juust out of reach, or eerily watching them from afar. Remember the Third Eye wants to haunt the players for some time, feeding on their fear, before trying to devour them with their souls. So you can introduce it against a low-level party, have it appear once in a while, silently watching them get stronger, before deciding that the time has come to put an end to the players.
